As a member of a Federal advisory committee, you are required to complete initial and annual ethics training.

Ethics Training

Under the Ethics in Government Act, the National Institutes of Health is required to provide ethics training to all newly appointed SGEs. The following are the training material links.

2. HHS-697 Form, “Foreign Activities Questionnaire” (FAQ)

The U.S. Office of Government Ethics (OGE) requires that all SGEs currently serving on NIH advisory committees complete an annual FAQ. Attached you will find the form and additional instructions. Please review, sign and return the FAQ.

3. NIH Advisory Committee Nominee/Member Certification Form (Lobbyist)

Federal policy now requires that Advisory Committee Members certify that they are not a federally registered lobbyist. Please review, sign and return the Lobbyist form.
